Embassy of the Republic of Lithuania (Washington DC) issues Lithuanian temporary travel document (Asmens grįžimo pažymėjimas) to Lithuanian citizens and current residents of Lithuania, only to come back to Lithuania, if their passport was lost, damaged or expired. Documents for Temporary travel documents have to be submitted either to the Embassy of the Republic of Lithuania (Washington DC) or Consulates General in New York or Chicago in accordance with their consular jurisdiction.
To obtain Travel Document, it is necessary to apply in person. If the applicant applies by mail, he/she must pick up the document in person. Only the U.S. Immigrations authorities can apply and receive the document by mail.
Travel document is valid for no more than 15 days from the date of issuance.

Application form filled in Lithuanian; 2 passport photos; Expired passport; if passport is lost form about lost document; Declaration on non-acquisition of foreign citizenship if passport lost or expired for more than 6 months; Sef-addressed envelope with paid postage; Consular fee.
